Electronic Funds Transfer (EFT)
Along with the Health Care Claim Payment/Advice transaction is the Electronic Funds Transfer (EFT). Completion of the EFT Questionnaire for HealthCare Providers and the Authorization Agreement for Direct Deposit of Payments forms along with a voided check from your business’ bank is required prior to implementation of the EFT process.

Prior to establishing an EFT connection with State Farm, a Provider must have a contract with WebMD to receive the ERA. To obtain an ERA contract, call WebMD at 800-215-4730. Providers who receive ERA’s through WebMD, will need to complete the ERA Setup Form to add a new payer (State Farm) to their existing setup. Contact WebMD at 800-845-6592 for information about the ERA Setup Form.
