You will need to agree to the Legal Disclaimer and Terms and provide your repairer ID number in order to complete and submit this form. If you do not know your repairer ID number, please contact your State Farm Estimatics Team Manager for assistance. Once the form has been received, it will be reviewed, and upon approval, you will be provided with a temporary id and password for additional access to the b2b site.
