Does State Farm's Health Division offer a Web Portal to view Eligibility, Claim Status, or Remittance data?

The Health Division does not support a Web Portal, however, we do support the HIPAA 270/271, 276/277 (through UHIN), and the HIPAA 835 (through Change Healthcare or UHIN).

I've received my EFT but cannot locate the ERA. What should I do?

First determine whether you still have a connection to our clearinghouse for the ERA, either Change Healthcare or UHIN. This would be either a direct connection or a connection through your clearinghouse. If you determine you no longer have a connection to one of our clearinghouses, please notify us via email at HLTH-MEDICAL PROVIDERS. Please provide your TIN, contact name, phone number and fax number. We will fax a paper copy of your EOB, and will remove you from the electronic process.

How do I Enroll for ERA and EFT?

You must have established a connection with one of the 3 clearinghouses Health has a connection with, Change Healthcare, UHIN, PNC Bank. Or you may choose to use the CAQH – CORE 270 Connectivity Rule. Enrolling with State Farm – Health will require you to access a secured B2B site. To register on the secure B2B site, please complete the Contact Us Health Division form.

How can I send my claims to State Farm Health?

State Farm Health (Medicare Supplement) can receive claims by any of these methods:

  • Electronically using the electronic healthcare claim transaction (ASC X12N 837)
